The TC have recently published a new Incident Report Form.

This form is designed to be easier to complete.

It is now available to download CFT 305a - Accident Report Form V4.1

The TC hopes that this new form will encourage all divers, but especially, Diving Officers, to report incidents to CFT.

Coupled with the new form is a new commitment to confidentiality. Unless the report has to go to CFT's insurers, only the National Diving Officer, the Incidents Officer and the office manager will see the report.

TC members will NOT see the report; Executive Council members will NOT see the report. Reports will NOT be circulated in any way.

I would encourage members to report incidents. This will allow CFT members countrywide to learn from incidents and to help us avoid similar incidents.

Finally, the Incidents Officer will report an annual report based on the incidents supplied by members.
