Incident Reporting

Incident Reporting

The sport of Scuba Diving involves immersion in an environment that traditionally does not support human life. We are also using bigger and faster boats and exploring parts of our coastline where few venture.

The risks in our sport are inherent and we must strive to make the standards safer for all. Incident reports have been recorded by CFT for many years and the lessons learnt from these incidents and near misses are important to CFT and of course to each and every member. It allows us to spot new risks in diving and boating and feed this information back into our courses, and "Rules and Recommendations". Our objectives with incident reporting would be:
  • Learning from our mistakes
  • Sharing information
  • Identifying training needs
  • Identifying areas where equipment is likely to fail
With the publication of annual incident reports, members will have a greater awareness of these issues. It is the responsibility of each club's Diving Officer to report an accident or a near miss in a timely manner.

We are working hard to make this process easier and to allay any fears or misapprehensions divers have with the whole process. We have made some progress in this regard which I would like to communicate to you:
  • Greater Web Presence: We now have an incident's page. I'm hoping to expand on this Incident's page and include a lot more content to do with Safety and Incidents.
  • New Accident Form: This will be easier to fill out. This new form will be ratified by the TC soon.
  • Confidentiality: We are changing the policy, so that only the NDO, Incident's Officer and CFT Office manager will have access to the names and club details of people involved. Where accidents do occur, they may be forwarded to our insurance broker.
  • Blame: This is not a process about blame, but to achieve our objectives stated above. Accidents and Near Misses do happen and very few divers can put their hand up and say they never had one.
  • Acknowledgements: Head Office and I will acknowledge all reports received.
